Insurance
Dr. Kaplan is currently an In-Network provider with Carefirst and Blue Cross Blue Shield. You are required to pay your co-pay at the time of service. Interns DO NOT accept insurance, though charge a reduced fee (see below).
For all other insurance plans, I am an Out-of-Network provider. This means that you will be required to pay the session fee at the time of service and then submit directly to your insurance company for reimbursement. If requested, I am happy to provide a monthly bill for you to submit to your insurance company.
While most insurance plans provide mental health service coverage, I would encourage you to contact your insurance company prior to scheduling a session to better understand your coverage plan.
Questions to ask your insurance company:
Does my plan include mental health benefits?
Do I have a deductible? If so, what is it and has it been met?
What do I need to do in order to receive reimbursement for therapy with an out-of-network provider?
What percentage of the session will be reimbursed?
Are there session limits?
Fees
Initial Assessment- $220
Individual Therapy- $165
Fertility Consultation for Donor Recipients- $350*
Initial Assessment with Intern- $60**
Individual Therapy with Intern- $50**
Payment is due at the beginning of the session. Cash, checks, and credit cards are accepted.
* Fertility Consultations are not typically paid by insurance, though I am happy to provide a superbill for you, as some insurance companies may reimburse you for the service. This fee includes the hour-long consultation, as well as time for report writing and collaboration with your fertility clinic.
** Interns can offer a sliding scale fee. Please inquire about this with your provider.
Late Arrivals & Cancellations
Clients are responsible for the full session fee if they do not give at least 24 hours notice prior to cancelling an appointment. Please note that insurance companies do not reimburse for late cancellation fees. Sessions are not prorated due to late arrivals.
